DementiaRoadmap

The Community Older Peoples Team (COPT) is made up of Health and Social Care staff including social workers, nursing, physiotherapy and occupational therapy. The team provide assessments and interventions to enable people to remain in the community and to support carers.

The service is for individuals aged 65 years and over who live in West Dunbartonshire and require input from Health and Social Care staff to remain at home, where possible

Anyone can refer to our service including yourself, family members, someone who is looking after you, health and social care professionals and voluntary organisations.

If someone is referring on your behalf we prefer they have your permission to do so.

Following an assessment the team can provide the help and support to adults and those who care for them, so that people can live as independently as possible in the community.

• Mobility – getting around
• Care at Home
• Daycare
• Exercise programmes
• Nursing care
• Respite
• Falls prevention
• Arranging packages of care and community alarms
• Healthy living
