My Life Assessment
If you are eligible for support from West Dunbartonshire Health and Social Care Partnership, a ‘My Life Assessment’ will be completed. This will identify the outcomes you require support to meet and will provide you with an indicative budget to use to do this.
Following an assessment, the team can provide help and support to adults and those who care for them so that people can live as independently as possible in the community. This may involve support from West Dunbartonshire HSCP home care services, other support providers, technology or a combination of these and other supports. The support planning process will inform this.
