Care at Home Services in West Dunbartonshire have begun implementing the process of all new referrals for “Care at Home” support. These referrals need to go through the duty social work team now who, in turn, will carry out their “My Life Assessment”. This is a screening tool that will confirm an assessment of needs for the individual.
Once the assessment is done, the social work team will place a referral on to the Care at Home Service clipboard via the CareFirst system, this enables all health professionals involved in the persons care to see what stage their assessment is at and when their home support is commencing.
