Day Care Services
Eligibility criteria would be people who have high level needs and are at a critical or substantial level of need and support; people may also be socially isolated and in need of stimulation.
Social Work and Home Care can refer via Care First to:
- Add an activity
- Offer additional input
- Day care services
Self-referral can be made by calling duty Social Work on 01389 776499.

An initial My Life Assessment is required prior to anyone commencing Day Services. Once referred, the staff from the Day Services will carry out a home visit, this is mainly to establish mobility status, what equipment is used, transport requirements and to carry out a risk assessment.
It is important that people have transport to and from the day centre of their choice. To facilitate a good transport service, it has been agreed to ensure a service is provided that offers much needed respite for families and carers. The buses will leave the centre each morning at 10am and leave to take clients home again at 5pm. There will be staff in the centre from 9am should families need to drop their loved one off early.
